Social Care Partners of Illinois is an operator-led network bridging Medicaid members to housing, food, health, and community support — through trusted local organizations across the state.
We work with community-based organizations, healthcare providers, and government agencies to deliver whole-person care — addressing the social determinants that drive health outcomes for Illinois Medicaid members.
What We Do
Six Domains of Social Care
Housing Stability
We connect Medicaid members facing housing insecurity to emergency shelter, transitional housing, rental assistance, and long-term stable housing solutions across Illinois.
Food Security
Our network links members to food pantries, SNAP enrollment support, meal delivery programs, and community nutrition resources to address food insecurity at the household level.
Community Health
We coordinate preventive care, chronic disease management, mental health navigation, and substance use support — meeting members in their communities, not just clinical settings.
Family Planning & Support
From prenatal care navigation to parenting resources and family counseling, we support Illinois families at every stage — with culturally responsive, trauma-informed care.
Justice-Involved Support
We provide reentry care coordination, benefits enrollment, and community connection for justice-involved individuals — reducing recidivism and improving health outcomes.
Care Coordination
Our care coordinators serve as the connective tissue of the network — building trust with members, navigating complex systems, and ensuring no one falls through the cracks.
