We're building a statewide network of trusted organizations committed to whole-person care for Illinois Medicaid members. If your organization provides social care services, we want to work with you.

Eligibility

Who Can Join the Network?

Community-Based Organizations

Nonprofits and CBOs providing housing, food, workforce, or social services to Illinois residents.

Healthcare Providers

FQHCs, community mental health centers, behavioral health providers, and primary care practices.

Social Service Agencies

Government agencies, faith-based organizations, and social service providers serving Medicaid populations.

Core Eligibility Requirements

Operates in Illinois and serves Medicaid-eligible populations

Provides services in one or more of our six social care domains

Maintains organizational liability insurance

Willing to participate in data sharing and outcome reporting

Able to accept electronic referrals via our platform

Committed to trauma-informed, culturally responsive service delivery

How It Works

The Partnership Process

Submit an Inquiry

Complete the partnership inquiry form below. Our network team reviews all submissions within 5 business days.

Discovery Call

We schedule a 30-minute call to learn about your organization, the populations you serve, and how we can collaborate.

Eligibility Review

Our team reviews your organization's services, capacity, and alignment with our Medicaid network requirements.

Partnership Agreement

Eligible organizations complete a partnership agreement and data-sharing addendum. We handle the paperwork.

Onboarding & Training

Your team receives onboarding to our referral platform, care coordination protocols, and reporting tools.

Go Live

Your organization is activated in the network and begins receiving referrals from care coordinators and MCOs.
